It could be done with Illustrator.

It was likely done with a vector art program like Adobe Illustrator or Adobe FreeHand.

According to the artist: "I draw my mandalas in Kali 1.0 and Mandala 1.0; both are OS7.5 programs. they only produce low-res bitmaps or PICTs. i can't even open them directly in Photoshop! i have to take a screenshot of them, then open the PDF file in Photoshop. they're jaggy and bitmappy at that point, so i apply a Gaussian blur of .3 and reduce them in size to no more than 6" wide. they're all 72 dpi."
