I'm trying to create a "bulge" under someone's clothing, as if they have something in a pocket or hidden under a jacket. Does anyone have experience with anything like this? Thanks in advance.

Have you tried using bevel and emboss for this? It would create a shadow and a highlight which you can then manipulate and deform to fit just about any form.

I've been tinkering with it, but I can't seem to get a realistic blend with the background.
